How to Receive Your Rebate!
- Get an eye exam. Product purchase must be made within 90 days of eye exam.
- Purchase required quantity of FreshLook®, Focus® Softcolors®,
Focus® DAILIES®, NIGHT & DAY®, or O2OPTIX™ contact lenses as indicated
above.
- Complete the mail-in certificate.
- Attach your original dated sales receipt (photocopies are not acceptable),
for the qualifying purchases and a copy of your valid eye exam receipt.
- Mail in the package UPC codes from half of the product purchase as
indicated above, along with your original sales
receipt indicating total purchase quantity (photocopies are not
acceptable), a copy of your eye exam receipt, and this Official
Mail-in Certificate to:

Rebate is limited during promotion period to one (1) offer per person per promotion period. Group or organization
requests are void. Rebates will be sent only to street or rural route addresses.
Requests from P.O. boxes will not be honored. Original mail-in certificate, eye exam receipt and sales receipt
are required; reproductions are not acceptable. Valid on purchases between
July 1, 2007 and January 31, 2008 and within 90 days of eye exam. All rebate requests must be submitted by February 28, 2008.
No late submissions will be accepted. Offer good only in the USA and Puerto Rico. Void where
prohibited by law. Please allow 6-8 weeks for processing.
