How to Receive Your Rebate!

1. Get a contact lens fitting and/or eye exam.
2. Purchase required boxes as indicated above.
3. Complete the mail-in certificate.
4. Save your boxes and all receipts for rebate submission (photocopies are not acceptable), for the qualifying purchases and a copy of your valid eye exam and/or contact lens fitting receipt.
5. Go to cibavision-rebates.com and enter your Individual Rebate Code
6. Download and complete the CIBA VISION® Official Rebate Form.
7. If you don’t have access to a computer, call 877-886-4321 to obtain your rebate. This will add two weeks to processing time.
8. Keep a copy of your paperwork for your records.
