AC Lens Vision Insurance Reimbursement Information

AC Lens is an Out-Of-Network provider for the following vision insurance companies:
The general procedure for reimbursement for these Insurance Providers is as follows:
  
  1. Fill out an Out-Of-Network Reimbursement Form.
  2. Include with the form an itemized receipt (AC Lens provides these with every purchase).
  3. Mail, Fax, or Email the completed form to the corresponding vision insurance provider.

Cole Managed Vision
   You should fill out and submit Out-Of-Network-Reimbursement-Form with itemized receipt to:

Spectera
   You will need to do the following steps to recieve your Spectera reimbursement.

  1. An itemized receipt
  2. Fill out an Out-Of-Network-Reimbursement-Form
  3. Subscriber's name, address, and Spectera ID#
  4. Patient's name and date of birth
  5. A note requesting reimbursement

VSP
   If you have an out-of-network benefit included in your plan, for reimbursement simply submit your itemized receipt from your order along with a VSP out-of-network reimbursement form to VSP. The reimbursement form can be found on VSP's website.
