AC Lens is an out-of-network provider for Davis Vision, EyeMed, Spectra/United Healthcare, VSP, and most other insurance plans.
The general procedure for reimbursement by these insurance providers is as follows:
- Fill out an out-of-network reimbursement form.
- Include your itemized receipt with the form (AC Lens provides these with every purchase).
- Mail, fax, or email the completed form to the corresponding vision insurance provider.
Some providers have their own online process. Make sure to check with your provider first to verify.
